In recent mine disasters,where does the carbon monoxide come from if there is no fire?

Coal waste left laying about will heat, sometimes to to smoldering, by spontaneous air oxidation. Finely divided pyrites contamination pushes that to fast forward. Folks who mine amidst organics (including oil shale) have hopcalite breathers on ther belts. They will save you for a few dozen breaths before the heat of oxidation burns your lungs.

Any source of combustion underground - explosion - is rich with CO.
