"Phone 11 Cold Well Drugs, 211 N. Penn Ave., Independence, Kansas, Directions Inside" on the box's lid. (no directions are included.) It also has a wishing well.
The box is white with blue trim.
Approximately 2.5" across x 1" high and the lid is 1.5" from its top to the bottom.
The lid is attached to the bottom of the box.
It was never used for a prescription but it does have some discoloration from age. Please view all photos.
