HIV transmissions seems to be greatly enhanced by other infections and these have always correlated to personal hygeine.

Perhaps also the amount of exposed skin? Perhaps places where residues can remain in contact with skin for longer periods of time?

I remember being part of studies on anabolic steroids many years ago where percutaneous penetration (the ability of the drug to penetrate skin) was greatly enhanced by covering the skin with saran wrap and thus increasing temperature and hydration. Might be a similar mechanism.
